This gradual information process is to be recorded in the patient's medical history. Occasionally, a document known as an “informed consent form” will need to be completed. This document states the risks inherent to the intervention or diagnostic procedure, the individual risks and possible side effects. This document is to be read carefully and then signed if they agree.
